The undersigned makes application for the installation and inspection of a sewer line and/or tap. It is the responsibility of the Applicant to inform all utility companies whose lines might be affected by the proposed construction. Sewer Permit fee is $25 and must be paid when the application is submitted. Following application submission you will be notified if permit is granted or denied. Work shall not commence until permit is granted. Once permit is granted and work is complete, inspection must be made and signed off on before back filling.
